Project Description


Revolutionising Primary Healthcare Access is a mobile triage and navigation app designed to improve access to primary healthcare services. The app features three integrated components: Health Check, an AI-powered chatbot that uses quick-response buttons to capture symptoms and guide patients to the most relevant care pathway; Find Care, a service navigator that ranks pharmacies, clinics, and hospitals by relevance, distance, and wait times; and My Health, a personal dashboard that connects with smartwatches to record vital health data, apply gamification to encourage healthy habits, and provide predictive risk assessments. Together, these features empower patients to access care faster, more accurately, and with less effort.

Data Story


Our solution reuses open health datasets from New Zealand, including the Health and Independence Report 2023, the New Zealand Health Survey, and Published Health New Zealand Data. These provide insights into healthcare access, inequities, and system inefficiencies that directly shape the app’s logic for triage, service matching, and patient monitoring. Location and provider datasets are used to build the “Find Care” navigator, while health survey statistics inform the AI chatbot’s risk guidance and predictive features. By combining real-world data with digital innovation, this project demonstrates how open data can directly reduce barriers to care, optimise service use, and relieve pressure on public health systems.
